Qualifying Criteria

Medicare Admission Requirements for Home Health:

• Patient is an eligible Medicare beneficiary
• Service must be ordered by a physician
• Service must be reasonable and necessary in the treatment of the illness or injury
• Patient must require a qualifying skilled service
• Patient must be homebound-see definition below

Homebound Definition

Patient is considered homebound if absences from the home are infrequent, require considerable and taxing effort, of short duration and are attributable to the need of:

• Medical Care
• Attend Adult Day Care
• Attend Religious Services
• Attend Beauty Shop
• Brief walk around the block
• A drive
