Step-by-Step Instructions

1. Click on the following link - https://familylink.hchb.com
2. If this is your first time to the website, please click on register.
3. Enter all required information to register.
4. Once registration is complete and you login, place your cursor over Administration and select Manage Patients.
5. Click on Add a Patient.
6. Enter American for Provider Code and hit submit.
7. Enter patient information. This includes patient’s first and last name, DOB MM/DD/YEAR and patient’s password.
8. Select Insert.
9. Click on Patient Information and choose between patient summary, diagnoses,medications, notes and vital signs to review the status of these categories.
10. Click on Journal to communicate with other family members and caregivers that the patient has given permission to access their information.
